According to Official Guinness Records,

The fastest long course swim in the 400 metres medley (female) is 4 min 26.36 sec, achieved by Katinka Hosszu (Hungary) at the 2016 Summer Olympics, in Rio de Janeiro, Brazil, on 6 August 2016.Katinka achieved this record in the final, which lead to her claiming her first Olympic gold medal.
